Lewis Moody MBE will join former teammates and supporters at Mattioli Woods Welford Road this weekend.
Saturday's Gallagher PREM match is the first meeting of his former clubs Leicester Tigers and Bath this season and comes 10 days after the former England captain announced his Motor Neurone Disease diagnosis.
It is a chance for him to thank some of the many fans who have sent messages of goodwill and donated to the GoFundMe page that will help support Lewis, his family and other charities.
He said: "I'm delighted to be attending this game, excited to see some old faces, support my two former clubs and return to my rugby home. I know it will be a special moment with the fans."
Some of Leicester's greatest players and teammates of Lewis — the Sabretooth Tigers — will be at the stadium and will gratefully collect donations, both cash and card, as well as posing for photographs and signing autographs.
Tigers have also made giving via GoFundMe accessible through their leicestertigers.com website and its ticketing platform, where donations can be added at checkout.
Tigers' Chief Executive Andrea Pinchen said: "It's a measure of the man that Lewis wants to say thank you for the support and affection the Tigers' family — and fans throughout the rugby world — have shown when his two clubs meet this weekend. We can't wait to welcome him back home.
"The Sabretooths have come forward to give their support, which means some of our greatest and most respected players will join us on Saturday to show their support, shake buckets and you might even see some of them show their talents under the high ball at half time.
"Please spread the word and come down to show your support for Lewis, Annie and their family, and donate if you can. No matter how big or small, your support does make a difference."
