Having won international honors at U16 level, Macauley was selected as a member of the 2008/9 Wales U18s squad.
In the summer of 2009 Cook was allocated to Pontypridd RFC as one of the club's quota of Blues Academy players but was transferred to Cardiff RFC by the Blues at the beginning of the 2010/11 season.
He made his senior Blues debut against the Exeter Chiefs in 2010. Macauley went on to make 12 appearances for the Blues in the 2011/12. Cook made his Heineken Cup debut for the Blues in 2012 against Montpellier.
In February 2015, Cook, who can operate at lock or back row, has now pledged his future by agreeing a new two-year extension to remain at BT Sport Cardiff Arms Park until at least 2018.
Cook is rated as one of the most exciting young talents in Welsh rugby and has become a key component in the pack at Blues.
In December 2017, Cook pledged his future to the Blues with a long-term contract extension. The versatile forward has established himself as an invaluable member of the Blues squad, making 137 appearances since his debut in 2010.
He heads into the 2018/19 season with 148 appearances for Cardiff having scored 85 points.
As of the 2018/2019 Champions Cup, Cook has had 30 European Cups appearances scoring 7 tries and 25 points.
